- Application for Assistance. (Every six months you must fill a new form)
- Application For Continuing Assistance (after your first visit and six month application, please use this application).
You will be asked to have your photo ID for yourself and any adult or child living in the home. For children, if they do not have photo ID’s, birth certificates, social security cards, and insurance cards are sufficient.
Your first visit is an intake where we gather information on what is happening in your household. Remember, the township has seventy two (72) hours to make a decision on your financial situation. Please have all of your information readily available.
You will be asked to provide a bank statement, this includes Chime, Cash-app, or any place you exchange money.. If you do not have a bank or cash account, you will be asked to fill out a form that you have no bank account. You may also be asked to provide receipts, paystubs, utility bills, and other bills to look into your financial situation. You may be asked for tax returns, lease and mortgage agreement, and or employment documentation. If you are not working, you may be asked to apply for jobs if you are able bodied.
